Processed food and ultra-processed food are not the same thing, and the difference between processed and ultra-processed food is probably the most misunderstood distinction in the entire UPF debate. Under the NOVA classification, a tin of chopped tomatoes and a jar of flavoured pasta sauce sit in different groups — and the health research making headlines is about the second group, not the first. The honest caveat: the group always depends on the specific product's ingredient list, so “typically” is doing real work in everything that follows.
UPF vs processed food: what is the difference?
NOVA, the framework developed by Carlos Monteiro and colleagues at the University of São Paulo, sorts food into four groups by the extent and purpose of processing — not by nutrients. Group 1 is unprocessed or minimally processed food: fruit, vegetables, plain flour, eggs, milk, plain yoghurt, meat, fish. Group 2 is culinary ingredients: salt, sugar, honey, butter, oils, vinegar. The confusion starts with the next two.
NOVA group 3 — processed food— is what you get when group 2 is added to group 1, chiefly to make food last longer or taste better. Usually two or three ingredients in total. Canned vegetables and legumes. Canned fish. Salted or smoked meats. Fruits in syrup. Freshly made, unpackaged bread. Traditional block cheese. These are recognisable foods with something ordinary added.
NOVA group 4 — ultra-processed food (UPF) — is different in kind, not just degree. These are industrial formulations made mostly or wholly from substances derived from foods, plus additives, with little or no intact whole food left. Soft drinks, packaged snacks, mass-produced sliced bread, most breakfast cereals, reconstituted meat products, instant noodles, many ready meals, confectionery.
So in the UPF vs processed food question, the answer is one of category, not severity: group 3 is cooking scaled up; group 4 is manufacturing.
NOVA group 3 vs 4: where the line actually sits
The line is drawn by markers of ultra-processing, and NOVA is specific about what counts. A food is group 4 when its label shows one of two things:
- Food substances with no or rare home-kitchen use — high-fructose corn syrup, invert sugar, maltodextrin, hydrogenated or interesterified oils, hydrolysed proteins, protein isolates, mechanically separated meat.
- Cosmetic additives— those whose purpose is to imitate or enhance the sensory qualities of the final product: flavourings and flavour enhancers, colours, non-sugar sweeteners, emulsifiers, thickeners, gelling, bulking and glazing agents.
Here is the nuance most coverage skips: additives used purely to preserve — preservatives, antioxidants and stabilisers added for durability — do not by themselves make a food ultra-processed. Monteiro and the FAO explicitly place fruits in syrup with added antioxidants and dried salted meats with added preservatives in group 3. One preservative is not proof of NOVA 4. An emulsifier, a colour, a flavouring or a protein isolate typically is.
And NOVA is a four-group category, not a graded scale. One marker puts a product in group 4; five markers do not make it “more” ultra-processed.
Side by side: UK examples on each side of the line
The same aisle in Tesco or Sainsbury's usually holds both groups within arm's reach of each other.
- Canned tomatoes vs jarred pasta sauce.A tin of chopped tomatoes — tomatoes, tomato juice, perhaps an acidity regulator — is group 3 territory. A jarred sauce that adds flavourings, thickeners or modified starch tips into group 4 — and plenty on the shelf do, so check the label. Some simpler sauces stay group 3.
- Block cheddar vs cheese slices.Traditional block cheese — milk, cultures, rennet, salt — is NOVA 3. Processed cheese slices and spreads add emulsifying salts and often colours, which makes them NOVA 4. Same word on the packet, different group entirely. - Bakery loaf vs wrapped sliced bread.Freshly made, unpackaged bread from flour, water, yeast and salt is group 3. Mass-produced packaged sliced bread — typically made via the Chorleywood process with emulsifiers and added fats — is usually group 4. The detail is in Is bread ultra-processed?
- Classic baked beans.The great surprise: beans in a tomato, sugar, salt and starch sauce are often group 3, not 4. They tip over only if the specific tin adds sweeteners, flavour enhancers or other markers — a good example of why reading beats assuming.
The pattern repeats across the shop. Plain oats are minimally processed; most breakfast cereals are group 4. A simple houmous can sit at group 3; a tub with gums and flavour additives lands in group 4 — see Is hummus ultra-processed?
The honest caveat: even experts disagree
NOVA's line is real, but it is not razor-sharp. In a 2022 study by Braesco and colleagues, over 150 experts classified the same foods and reached only “fair” agreement — a Fleiss kappa of about 0.32 for marketed foods and 0.34 for generic foods. The same product can land in different groups depending on who codes it and how much ingredient detail they have.
The UK's Scientific Advisory Committee on Nutrition takes a similarly measured view. Its 2023 position statement found NOVA the only processing classification potentially suitable for UK use, but flagged concerns about its consistency and said the largely observational evidence should be treated with caution. The large studies linking UPF to poor health outcomes show association, not proven causation. Ultra-processed and unhealthy are correlated, not synonymous — a fortified wholemeal loaf can be NOVA 4 and still be a decent choice.
None of that makes the group 3 vs group 4 distinction useless. It makes it one useful signal among several — which is exactly how it should be read.
What to check on the label
You do not need to memorise the framework. Three questions cover most of it:
- Could you buy every ingredient in a supermarket as a standalone item?Flour, oil, salt, tomatoes — yes. Maltodextrin, protein isolate, invert sugar — no. The “no” column is the group 4 signal.
- Are there cosmetic additives? Flavourings, colours, sweeteners, emulsifiers and thickeners point to group 4. A lone preservative, antioxidant or acidity regulator does not.
- Is it still recognisably the original food?Beans in sauce, fish in brine, milk turned to cheese — typically group 3. A formulation reassembled from derivatives — group 4.
